An investigation is being launched after a Washington truck driver crashed his 18 wheeler into a river last Thursday, ultimately killing him.

So far Police suspect brake failure, as no skid marks were evident at the scene of the crash.

Members of the local media reached out to the truck driver’s reporting company, but they declined to comment on the situation.

Leaders from Washington State Police said they’re waiting on a toxicology report and a vehicle inspection report to get a better idea of what may have caused this accident.
