The video was posted on YouTube and shows a manufacturing error on one of the ears of a brake pad. The defective surface prevented the DIYer from installing the brake pad in the caliper bracket. Typically these types of defects are caught by the press operator, worker mating the friction material to the backing plate or as part or quality control inspector. But, this pad managed to get past all those checks and almost onto the road. As to what caused the defect, it could be a worn-out die on the press or a piece of metal debris in the tooling. You should never try to correct these types of defects with a grinder, send the pads back.
