Update: Brake Failure Responsible For Canadian Crash

Back in December, we covered a story about a fifty year-old woman who was struck by a sign post and killed due to a runaway transit bus that originally smashed into it. Just under a month later, the Saskatchewan Coroner’s office has determined the death was completely accidental.

According to the coroner’s report, the bus was travelling at 22 km/h as it approached the bus stop. The driver applied the brakes, but the left one failed and caused the bus to swerve to the right, leave the roadway and then strike the sign post.

In addition to the failed left brake, the coroner also added that the bus was an older model lacking an ABS system. As a result, when the driver applied the brakes, the steering also locked as the bus skidded into the sign pole. The coroner said the driver should have released and pumped the brakes.

The Regina Police Service also investigated the incident and didn’t charge the driver. But on Aug. 13, the RPS did issue the city a $770 fine under Vehicle Equipment Regulations for failing to maintain “a brake system with mechanical components that are secure, functional or not excessively worn or misaligned.”
